Core-Dr Falk 2018 SpR Audit/Quality Improvement Awards Application Form

Dr Falk Pharma and Core invite applications from UK-based gastroenterology/hepatology SpR trainees who would like to conduct an audit or quality improvement project in any area of gastroenterology, liver disease or nutrition. The maximum award is £10,000.

Applications should be completed through the form below. A PDF version of your application will be sent to your email address after you submit the form (check your spam/junk folder if you have not received it). Please print this off, obtain all relevant signatures (Applicant, Supervisor and Finance Officer signatures required) and email a scanned signed full application to research@corecharity.org.uk by 2 March 2018, 5pm. The application will only be considered complete once we receive the signed version. Applications received after this time will not be considered.

Please do not exceed the word limits set in the application form and ensure all rich text formatted sections use 11 size font.

  • Section Two - Project Details

  • Maximum 30 words. Please choose an informative title.
  • Please indicate a realistic start date.
  • Indicate the primary host institution and additional sites, if applicable.
  • Word count between 1,000-1,500 words (excluding references). Please describe the background, aim/purpose, a brief outline of the project, and who will benefit from the outcomes. Add relevant references if needed (maximum 20).
  • Maximum 200 words. Use simple lay terms to describe the project and include the following:
    • A simple heading (maximum 10 words)
    • The audit/quality improvement area you are addressing, e.g., information on the condition(s)/service/treatment
    • How this project will benefit patients in the near future

    The score should be more than 50.
    If “No” please note that proof of ethics approval will be required before any funds are released.
  • Section Three - Budget

    Please note that the maximum award is £10,000 and applications requesting a larger amount will be rejected.
    Capital items (for example, laptops and other equipment) will not be funded under this grant scheme nor will any contribution to salary costs or higher degree fees for the applicants. Conference-related costs are not eligible for funding. Travel costs related to conducting the project can be included (for example site visits, focus groups etc.). All costs must be in UK pounds (£).
    Please ensure the Finance Officer has sufficient time to review the application budget and associated costs.
